Welcome to the Carmel High School Nurses' Office

Please be advised that New York State Requires that all ninth and eleventh grade students receive a physical examination.

Your family physician is best informed about your child's health; therefore we encourage parents/guardians to have the family physician perform this examination. In the event there is no family physician or if parents/guardians prefer the examination be done in school, the school nurse can arrange to have your child examined by our school physician.

The NYS Health Examination form is required and must be completed by a physician and submitted to the Carmel High School Nurse's office on or before the first day of school.

  • FamilyID registration for Interscholastic sports

All athletes must be registered each athletic season on FamilyID.  Once your athlete is registered you will receive an email advising you if your child has been approved, or not approved.  If they are not approved, there will be an explanation.  You don't need to call the nurse's office to see if your athlete has been approved or not, you will get an email.

For any other questions, please feel free to contact the Director of Athletics, Susan Dullea.

  • If I need to pick my child up early, what should I do?

If you need to pick your child up early you should send him/her in with a note explaining why you need to pick him/her up & the time you need them.  Your child  should bring the note to the main office upon entering the building.  He/she will be given a pass excusing him/her from class at the specified time. Your child will then meet you at the circle. In the event of an emergency, when you have not sent in a note, go to the main office. One of the secretaries will page your child out of class. The attendance will be adjusted accordingly by the end of the day or, at times, the next day.
